Methods For Using "On Line Fill In" Forms

"On Line Fill In" forms can be used several ways.  Choose the method that is easiest for you.

 

Fill Out "On Line" and Send As Attachment By E-Mail

1.

Open Form by clicking on it,

2.

Fill out form using your keyboard.  Use tab key to move from field to field,

3.

When finished, click on the "Down" arrow and save the form to your computer (Be sure to remember the name of the file, and the name of the folder where you saved it),

4.

Send the form to the E-Mail address on the form by attaching it to the E-Mail.

5.

You may, if you wish, print a copy of the file for your records, or you may depend on the "Saved" copy in your files.

 

 

 

Fill Out "On Line", Print, Scan, and Send As Attachment By E-Mail

1.

Open Form by clicking on it,

2.

Fill out form using your keyboard.  Use tab key to move from field to field,

3.

When finished, print the form,

4.

Use your scanner to scan the printed copy, and send that by attaching it to the E-Mail and sending that to the E-Mail address on the form.

 

 

 

Fill Out With A Pen, Scan, and Send As Attachment By E-Mail

1.

Open Form by clicking on it,

2.

Print the form,

3.

Fill out the form with a medium point pen,,

4.

Use your scanner to scan the filled in copy, and send that by attaching it to the E-Mail and sending that to the E-Mail address on the form.

 

 

 

Fill Out With A Pen and Mail Form

1.

Open Form by clicking on it,

2.

Print the form,

3.

Fill out the form with a medium point pen,

4.

Mail that to the address on the form.
